A health care surrogate designation is a legal document, written or oral, that designates another person as your representative to make medical decisions for you, if you are unable to make them for yourself. A health care surrogate document must be signed and witnessed.
It is similar to a living will, however, with a living will, you are making the medical decisions in advance, whereas with a health care surrogate designation, you are designating someone to make decisions for you, in the event that you become incapacitated.
Additionally, with a health care surrogate designation, you can outline any directions about any specific treatments or life-prolonging measures that you do or do not want, as with a living will. Once completed, it should be shared with your spouse, family members and medical providers, to make everyone involved in your care aware of your decisions.
